Because these substances usually are not detectable on standard urine toxicology screens, clinicians need to be aware of them to be able to take an accurate substance use history, consider possible dangerous interactions with prescribed psychotropics, and address medical and psychiatric complications.

At central nervous system receptor level, DMT interacts with serotonergic neurotransmission due to its structural similarity with the endogenous neurotransmitter serotonin 5-hydroxytryptamine; 5-HT Furthermore, DMT is an nerbal for 5-HT2A and 5-HT1A receptors sites [ 12 ]. Evidence also suggests that Kratom might be a deadly substance when mixed with other compounds [ 499495 - 98 ].
Synthetic cannabis, party pills, herbal highs: “psychoactive substances”

Ayahuasca is usually prepared by boiling or soaking parts of the above reported plants [ 6 ]. Several fatalities and unexpected sudden cardiac death have been related to iIbogaine consumption [,].
But in recent months new herbs like Salvia, a rare Mexican plant related to sage, and Kratom, an obscure Thai herb, have produced legions of devoted users, many of whom claim they are just as effective as illicit drugs. In the past kava-kava played an important role in different religious and political ceremonies, and still today it is consumed to restore the physical resistance, help sexual performances, acts as an aphrodisiac, alleviate stomach pain and many other indispositions.
The two botanical varieties are Nervosa and Speciosa; for both, the whole plant has been used in traditional ayurvedic medicine [ 23 ]. The US Drug Enforcement Administration includes the plant in its list of 'drugs and chemicals of concern' and has spent the past two years monitoring its use.
A case of persistent psychosis characterized by echolalia involuntary repetition of words or phrases spoken by othersparanoia, agitation and conflict of ideas was described in a young man of 21 years without past mental disorders who had smoked Salvia divinorum [ ]. At smaller doses, it is appreciated due to its stimulant and aphrodisiac properties  Common route of administration is oral.

If on the one hand the neuropharmacological effects of some of them are well established, on the other, one of the most important gigh which still needs clarification is the risk of addiction due to the abuse of these compounds.
